How to Register
If you live in the area and wish to register we will ask you to complete an NHS Registration Form and a practice Health Questionnaire. A member of the team will check the information and, if needed, you may wish to or be invited to make a registration appointment.
You can find details of our practice catchment area by clicking on practice boundary checker. Enter your postcode in the top right hand 'postcode' box to see whether you live within the practice area.
Before your registration can be accepted you will need to provide us with 2 forms of identification. Examples of acceptable ID include a driving licence, passport, a recent utility bill (e.g. gas, electric, satellite, TV, phone bill) issued within the last 3 months, a bank statement, council tax bill for the current council tax year, mortgage statement.
At the current time please contact either surgery if you wish to register or complete a registration form by clicking the link in purple and bringing it to either surgery.
We are unable to accept new out-of-area registrations at this time. If you are already registered with us and currently live outside of our catchment area you may receive a letter advising you of a need for a change of status and an out-of-area form to sign.
