How to register as a new patient

 

A warm welcome to new patients at Downend Health Group

If you live in the area and would like to register with us, please download and complete the forms below: 

 

Step One:    NHS Registration Form (GMS1)  Download and complete

Step Two:     Practice Health Questionnaire Registration Questionnaire     Download and complete

                         or

                       Easy Read Registration Questionnaire         

                        We have created an easy read version for our patients who prefer this format

 

Alternatively, you can collect  paper versions from our reception desk.

 

 

At this time, we can only accept new registrations for patients that live within our practice boundary. Please click on the link below and enter your postcode in the box located in the top right hand corner to check whether you live within our practice area.

Unfortunately, we currently cannot accept new registrations for patients who do not live within our catchment area. Further information relating to 'Out Of Area' registrations can be found in the link below. 

Registering with a GP Practice outside the area you live

If you are already registered with us and currently live outside of our catchment area you may receive a letter advising you of a need for a change of status and an out-of-area form to sign.

Once completed, please bring your forms to the surgery, along with 2 forms of identification, Acceptable forms of identification include: 

  • Driving licence

  • Passport

  • A utility bill issued in the last 3 months (gas, electric, telephone)

  • Mortgage statement

  • Bank statement

 

 

Proof of identification ensures that we match new patients to their previous surgery, ensuring the smooth transfer of your medical records. However, everyone has the right to free healthcare and if you do not have proof of identification or address we will can still register you. 

On receipt of the registration forms,  we will start the registration process for you and if needed, send you an invitation to make a registration appointment.

We allocate a 'Named, Accountable GP' to all of our registered patients. This GP will oversee any associated paperwork or test results. There may be occasion when, without prior notice, the practice may need to change your named, accountable GP. Please be assured that this has no effect on appointment availability, your treatment, or which of our clinicians you see. You are also not restricted to seeing your named, accountable GP, and are welcome to request any member of our healthcare team of your choosing.

Temporary Patients

We offer our services to individuals residing within our practice area for less than 3 months. If you are temporarily living in the area and require immediate or necessary care, please call us for further information and details on completing a temporary resident form.
